1/16/2024 EnchantaVaughn Approved 1.Behavior: Upon arrival, Laylah exhibits a good attitude and seem to be more aware of what she needs to work on and plans to utilize the acquired skills taught by QMHP-C daily. Mother reports when Laylah cannot have her way, she will storm out the room with behaviors lasting for 10 minutes in duration. 2. Behavior: Laylah shows great improvement with all objectives, identifying her triggers, recognizing unwanted behaviors, and refraining from aggression in the home as mother reports behaviors of storming utilizing profanity last about 10 minutes in duration. 1.Intervention: QMHP-C conducted a Q&A, following open discussion with Laylah and parent by assessing her behaviors and mood and asking questions to motivate Laylah to see a new perspective. QMHP-C inquired about Laylahâs current feelings and provided positive affirmations. QMHP-C probed about any angry feelings or the inclination of safe and unsafe behaviors. QMHP-C utilized active listening as information was conveyed. QMHP-C communicated with Laylah on displaying appropriate control. QMHP-C probed Laylah about certain behaviors and how she sometimes feels like the only way to respond is in a negative manner. QMHP-C continued to engage with Laylah and provided positive reinforcement as she participated in the session. QMHP-C challenged Laylah to identify any situation she has experienced that makes her feel defeated or misunderstood. QMHP-C further explained different situations that can cause anxiety. 2. Intervention: QMHP-C provided feedback regarding Laylah inquiries. QMHP-C continued to elaborate on ways Laylah can cope and manage stressors. QMHP-C commended Laylah for her participation and honesty. QMHP-C educated Laylah that most children her age are not as mature as her and that alone gives her an advantage and confirmation that sheâs headed in the right direction and to believe in herself. QMHP-C processed Laylah positive behaviors. QMHP-C assisted Laylah in developing more positive anti-social skills. QMHP-C inquired about possible challenges Laylah can experience with applying this skill. QMHP-C provided positive feedback. QMHP-C reflected on Laylah goals and objectives. QMHP-C isolated Laylah from distractions to decrease disruptive behaviors and to reduce episodes. QMHP-C assisted Laylah in normalizing the changes that are occurring in the home and in the community to prepare her for uncertainty and a new normal. 1. Response: Laylah answered a lot of the questions from the discussion in a calm, straight forward manner. Laylah responds to the Q&A with positive comments. Laylah expressed changed behavior which allowed her to see a new perspective, because of this Laylah demonstrated moderate progress during the session. Laylah was able to communicate with QMHP-C. Laylah communicated that she felt safe and comfortable talking to her in-home counselor. Laylah acknowledged that she has not become angry all day and continued to smile throughout the session. Laylah expressed she was happy to finally have someone she can talk to about her feelings both regarding family and friends. Laylah was receptive and open to the interventions, as she was attentive and focused on the information and activities that were being processed without any difficulties. Laylah appeared to have a clear knowledge of the interventions. 2. Response: Laylah was sociable and friendly in regard to QMHP-Câs positive empathy. Laylah was able to express and explore her perceptions of relationship skill building. Laylah was compliant in todayâs session, as evidenced by following directions. Laylah made eye contact towards QMHP-C, as she smiled with satisfaction and confidence. Laylah was monitored complying with QMHP-Câs positive and empathetic acceptance. Laylah was open to the mutual and trusting relationship that was reestablished by QMHP-C. Laylah displayed appropriate understanding of exercise. Laylah informed QMHP-C that she was happy and excited to come to session. Laylah said she enjoyed being around people that treat her nicely. Laylah understands that in order to do her best, she has to feel her best as well. Laylah displays appropriate attitude during the session, indicating she was learning and having fun. Laylah loved expressing herself around her mom and QMHP-C, as demonstrated by her high verbalizations and commitment with answering each question out loud. Mom was satisfied and content in reference to the next session with QMHP-C.
